Group Change Form
This form is designed to inform our General Service Office of a change to an existing group (i.e. name or location) and to keep the GSO apprised of new General Service Representatives (GSRs) and their Alternates. When your group elects a new GSR or Alternate-GSR, we recommend filling out this form. They will be provided helpful AA literature at no additional cost!

District Change Form
This document informs the General Service Office of new District Committee Members (DCMs). It is a valuable tool for helping Area record keepers know who the voting members of the Area are, and provides useful information for confidential contact lists. As with the GSRs, incoming DCMs will be provided helpful AA literature at no additional cost!
